A letter from Leslie W. Miller of the Fairmount Park Art Association requesting permission to erect a bronze memorial tablet designed by Zantzinger, Borie, and Medary (1915), a description of the tablet submitted to the Art Jury, a letter from Art Jury secretary Andrew Wright Crawford acknowledging receipt of the submission and accompanying blueprints (1915), and a Fairmount Park Commission resolution to erect the tablet in Memorial Hall (1915).
